Our RN-coached caregivers help prevent hospital readmission by supporting a safe recovery at home.

Yes, same-day discharge care is available in most cases. We strongly recommend contacting Mae's Grace 1-2 days before discharge to allow time to match the right caregiver and develop a care plan.
